This is no surprise at all. Anytime you go to a new practitioner, you have to fill out new paperwork. Your forms here will consist of new patient paperwork, so that we can learn more about you and your vision. We will also need your vision insurance information.

Health History Review

While you’re in the office with your eye doctor, they will review your health history form. During this time, they will go over any concerns regarding your eye health that you may have, as well as discuss other health conditions you may have that can affect the ocular health. Even seemingly unrelated concerns such as rosacea or Lyme disease can lead to vision problems, so it is important to fill out your paperwork accurately.

Complete Eye Exam

A comprehensive eye exam will be completed to measure the sharpness of your vision, and check for color blindness, ability to focus, ocular motility, blind spots, and / or depth perception. Your pupils may be dilated, so the doctor can better look into your eyes. The doctor will check your eyes for vision problems such as cataracts, amblyopia, eye strain, tumors, macular degeneration, and other conditions. They will check for myopia (nearsightedness) and hyperopia (farsightedness). If you need vision correction, they will use an auto refractor or aberrometer to help determine your prescription.

If you don’t need vision correction through eyeglasses or contacts, we will schedule you for your next appointment in either one or two years, depending on any eye conditions you may have. For those that require glasses or contacts, we will guide you to our optical center, where one of our associates will help you choose eyewear that’s right for you.
